General Provisions
This document constitutes an official public offer made by SIY Dental Clinic for the provision of dental services to individuals.
By using the website, booking an appointment through any available method, or receiving services from the Clinic, the Patient confirms acceptance of the terms and conditions of this Agreement.
Subject of the Agreement
The Clinic provides dental services in accordance with the applicable legislation of Ukraine and accepted medical standards, while the Patient agrees to pay for the services provided in a timely manner and follow the dentist’s recommendations.
Appointment Booking
Appointments for consultations or treatment may be booked through the website, by phone, via messaging applications, or directly at the Clinic.
If necessary, the Clinic may contact the Patient to confirm or reschedule the appointment.
Service Fees and Payment
The cost of dental services is determined according to the current price list of the Clinic.
The final cost of treatment may be determined after the consultation, examination, and preparation of an individual treatment plan.
Payment shall be made using a method agreed upon by the Clinic and the Patient.
Rights and Responsibilities of the Parties
The Clinic undertakes to provide dental services of appropriate quality, ensure the confidentiality of personal data, and provide the Patient with all necessary information regarding the proposed treatment.
The Patient undertakes to provide accurate information about their health condition, follow the dentist’s recommendations, comply with the Clinic’s rules, and pay for the services received in a timely manner.
Liability
The Clinic shall not be liable for any consequences resulting from inaccurate information provided by the Patient regarding their health condition or from failure to follow medical recommendations.
Both parties shall be liable in accordance with the applicable legislation of Ukraine.
Personal Data Protection
Patients’ personal data is processed in accordance with the Clinic’s Privacy Policy and the applicable legislation of Ukraine.
Final Provisions
The Clinic reserves the right to amend the terms of this Agreement at any time. The current version of the Agreement is always available on the website.
Any disputes shall be resolved through negotiations. If no agreement is reached, disputes shall be settled in accordance with the applicable legislation of Ukraine.
